Aging and Independence’s Chronic Disease Self Management program,
Senior Care for in-home assistance, and the local 50 Plus center for
socialization, and connected to the local transportation system.
Mr. and Mrs. G. are 72 years old and have owned their home in
Columbia for 40 years.They would like to remain in Howard County,
probably in their home but are open to other alternatives.They would
like to discuss their options and call Maryland Access Point. Mr. and
Mrs. G. decide they want to meet the MAP Specialist at Gateway
Office for Options Counseling.The Specialist discusses needs, available
supports, preference, strengths, and values with Mr. and Mrs. G.What
they would really like is to stay in their home for as long as possible.
Fortunately, they have saved for their retirement, have a good monthly
income, and have equity in the home. One thing they have not done
in many years is to update their wills and they do not have an Advance
Directive.Together with the MAP Specialist, they outlined a plan of
action. Items on the plan include 1) updating their wills 2)talking with
their adult children about their medical care wishes and preparing an
Advance Directive and a Medical Orders for Life Sustaining Treatment
(MOLST) 3) talking with their children and determining trigger
points for the time when they might need to purchase in-home
care 4) consulting with contractor on needed changes to make their
home aging-friendly 5) applying for the Livable Homes Tax Credit
6) considering a Reverse Mortgage to use the equity in the home to
make repairs 7) perhaps joining The Village in Howard or the Columbia
Association Community Exchange Time Bank to invest in the now as
giving back as volunteers or helping neighbors and then utilizing help
for themselves in the future.
